First Visit

Your first appointment is dependent upon your particular dental needs.

• Should you have an emergency, we will do our utmost to see you promptly and address your problem.
• If you need a dental cleaning with our hygienist and have been to our office in the past year, you will be offered our next available opening.
• If you are a new patient to our practice, we will schedule a New Patient Visit with either Dr. Hochberg or Dr. Friedman. At that time your medical and dental history will be reviewed; x-rays, if needed, will be taken; a complete oral examination will be completed and your dental treatment needs and expectations will be discussed.

Prior to visiting us a New Patient Welcome Packet will be sent to you along with a health history to complete.

Please assist us by providing the following information at the time of your appointment:

• Your completed health history form
• Your referral slip and any x-rays (if applicable)
• A list of your current medications
• If you have dental insurance, bring the information necessary for claim submission (policy and group numbers, insured’s ID number and the insurance company’s mailing address and phone number). This will save time and allow us to help you process any claims.

X-Rays: If your previous or referring dentist has taken x-rays, you may request that they send or email them to our office. If additional x-rays are necessary, they can be taken at our office.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the first visit.
